Common signs after a surfing impact — board to the head, wipeout with a hard bottom hit, or a collision with another surfer — include headache, dizziness, nausea, ringing in the ears, blurred or double vision, sensitivity to light or sound, feeling "foggy" or slowed down, trouble remembering the wave or the moments after, and unusual emotional reactions (irritability, tearfulness). You do NOT have to lose consciousness to have a concussion. If any of these show up, get out of the water, don't drive yourself home, and have someone evaluate you. When in doubt, sit out — repeated hits before you're fully healed are how mild concussions become long-term problems.

What is second impact syndrome, and why is it dangerous for surfers?

Second impact syndrome (SIS) happens when someone sustains a second concussion before the brain has fully healed from the first. Even a mild second hit can trigger rapid, catastrophic brain swelling — SIS has a very high rate of severe disability or death, and it disproportionately affects young athletes. Surfing is a uniquely high-risk environment for SIS because a concussed surfer often paddles back out the same day, faces repeated wave impacts, and may hit the reef, sand, their own board, or another surfer. Full symptom resolution and clearance to return is non-negotiable — the ocean will not wait for your brain to heal, so you have to.

Every concussion increases the risk of the next one — and increases the likelihood of long-term issues like Post-Concussion Syndrome, chronic traumatic encephalopathy (CTE), depression, and cognitive decline. Surfers who have had multiple concussions, especially close together, or who take longer to recover each time, should be evaluated by a specialist and consider whether continued high-impact surfing is worth the risk. Helmets, respecting crowded lineups, learning to fall flat, and giving each concussion full time to heal are the biggest levers surfers have.
